Despite the advancements in thrombolysis and thrombectomy facilitating proximal large artery recanalization, the resultant distal hypoperfusion, referred to “no-reflow” phenomenon, often impedes the neurological function restoration in patients. Over half a century of scientific inquiry has validated the existence of cerebral “no-reflow” in both animal models and human subjects. Furthermore, the correlation between “no-reflow” and adverse clinical outcomes underscores the necessity to address this phenomenon as a pivotal strategy for enhancing AIS prognoses. The underlying mechanisms of “no-reflow” are multifaceted, encompassing the formation of microemboli, microvascular compression and contraction. Moreover, a myriad of complex mechanisms warrant further investigation. Insights gleaned from mechanistic exploration have prompted advancements in “no-reflow” treatment, including microthrombosis therapy, which has demonstrated clinical efficacy in improving patient prognoses. The stagnation in current “no-reflow” diagnostic methods imposes limitations on the timely application of combined therapy on “no-reflow” post-recanalization. This narrative review will traverse the historical journey of the “no-reflow” phenomenon, delve into its underpinnings in AIS, and elucidate potential therapeutic and diagnostic strategies. Our aim is to equip readers with a swift comprehension of the “no-reflow” phenomenon and highlight critical points for future research endeavors.</jats:p>"}]}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1380025437249193344","@type":"Researcher","foaf:name":[{"@value":"Yang Zhang"}],"jpcoar:affiliationName":[{"@value":"Department of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193351","@type":"Researcher","foaf:name":[{"@value":"Miaowen Jiang"}],"jpcoar:affiliationName":[{"@value":"Beijing Institute for Brain Disorders, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193347","@type":"Researcher","foaf:name":[{"@value":"Yuan Gao"}],"jpcoar:affiliationName":[{"@value":"School of Instrumentation and Optoelectronic Engineering, Beihang University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193346","@type":"Researcher","foaf:name":[{"@value":"Wenbo Zhao"}],"jpcoar:affiliationName":[{"@value":"Department of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193352","@type":"Researcher","foaf:name":[{"@value":"Chuanjie Wu"}],"jpcoar:affiliationName":[{"@value":"Department of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193350","@type":"Researcher","foaf:name":[{"@value":"Chuanhui Li"}],"jpcoar:affiliationName":[{"@value":"Department of Neurosurgery, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193353","@type":"Researcher","foaf:name":[{"@value":"Ming Li"}],"jpcoar:affiliationName":[{"@value":"China-America Institute of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193348","@type":"Researcher","foaf:name":[{"@value":"Di Wu"}],"jpcoar:affiliationName":[{"@value":"China-America Institute of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193345","@type":"Researcher","foaf:name":[{"@value":"Wu Wang"}],"jpcoar:affiliationName":[{"@value":"Institute of Diagnostic and Interventional Radiology, Shanghai Sixth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine, Shanghai, China"}]},{"@id":"https://cir.nii.ac.jp/crid/1380025437249193349","@type":"Researcher","foaf:name":[{"@value":"Xunming Ji"}],"jpcoar:affiliationName":[{"@value":"Beijing Institute for Brain Disorders, Capital Medical University, Beijing, China"},{"@value":"Department of Neurosurgery, Xuanwu Hospital, Capital Medical University, Beijing, China"},{"@value":"China-America Institute of Neurology, Xuanwu Hospital, Capital Medical University, Beijing, China"}]}],"publication":{"publicationIdentifier":[{"@type":"PISSN","@value":"0271678X"},{"@type":"EISSN","@value":"15597016"}],"prism:publicationName":[{"@value":"Journal of Cerebral Blood Flow & Metabolism"}],"dc:publisher":[{"@value":"SAGE Publications"}],"prism:publicationDate":"2023-10-19","prism:volume":"44","prism:number":"1","prism:startingPage":"19","prism:endingPage":"37"},"reviewed":"false","dc:rights":["https://journals.sagepub.com/page/policies/text-and-data-mining-license"],"url":[{"@id":"https://journals.sagepub.com/doi/pdf/10.1177/0271678X231208476"},{"@id":"https://journals.sagepub.com/doi/full-xml/10.1177/0271678X231208476"}],"createdAt":"2023-10-19","modifiedAt":"2025-08-07","relatedProduct":[{"@id":"https://cir.nii.ac.jp/crid/1360023718185545984","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@value":"Recanalization Does Not Always Equate to Reperfusion: No-Reflow Phenomenon After Successful Thrombectomy"}]},{"@id":"https://cir.nii.ac.jp/crid/1360588380155779200","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@value":"Oxygen–glucose-deprived peripheral blood mononuclear cells act on hypoxic lesions after ischemia-reperfusion injury"}]},{"@id":"https://cir.nii.ac.jp/crid/1360588380606876672","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@value":"<scp>CHA<sub>2</sub>DS<sub>2</sub></scp>‐<scp>VASc</scp> score and prior oral anticoagulant use on endovascular treatment for acute ischemic stroke"}]}],"dataSourceIdentifier":[{"@type":"CROSSREF","@value":"10.1177/0271678x231208476"},{"@type":"CROSSREF","@value":"10.1161/strokeaha.124.048994_references_DOI_NAm7Xji3yJA4fr8AVE90o5ZL3uF"},{"@type":"CROSSREF","@value":"10.1016/j.expneurol.2024.115121_references_DOI_UtNIYtU5cNxk6WhYYjpOPzjQg3i"},{"@type":"CROSSREF","@value":"10.1002/acn3.52217_references_DOI_UtNIYtU5cNxk6WhYYjpOPzjQg3i"}]}
