The patients were randomly assigned to either a strict-treatment group (target systolic blood pressure (BP)140 mm Hg, n=2212) or a mild-treatment group (target systolic BP, 140 to160 mm Hg, n=2206), each with efonidipine (a T/L-type Ca channel blocker)-based regimens. Cardiovascular events (stroke, cardiovascular disease and renal disease) were evaluated during the 2-year follow-up period following the prospective randomized open-blinded end-point method. Estimated glomerular filtration rate (eGFR) was elevated throughout the trial period in both the strict-treatment (59.4-62 ml min⁻¹ per 1.73 m²) and the mild-treatment group (58.8-61.4 ml min⁻¹ per 1.73 m²). This tendency was also observed in diabetic patients and patients aged ≥75 years, with baseline eGFR60 ml min⁻¹ per 1.73 m². Baseline eGFR (60 vs. ≥60 ml min⁻¹ per 1.73 m²) had no definite relationship with the incidence of cardiovascular events, nor did the level of BP control. Proteinuria at the time of entry into the study, however, was significantly correlated with cardiovascular event rates (7.1%), an association that was more apparent in patients with eGFR60 ml min⁻¹ per 1.73 m² (8.2%). Furthermore, the event rate was more elevated in patients with greater declines in eGFR and was amplified when the baseline eGFR was60 ml min⁻¹ per 1.73 m². In conclusion, the rates of decline of renal function and proteinuria constitute critical risk factors for cardiovascular events in elderly hypertensive patients, trends that are enhanced when baseline eGFR is diminished. Furthermore, the fact that efonidipine-based regimens ameliorate renal function in elderly hypertensive patients with chronic kidney disease may offer novel information on the mechanisms of cardiovascular protection."}]}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1382825895359767296","@type":"Researcher","foaf:name":[{"@value":"Koichi Hayashi"}]},{"@id":"https://cir.nii.ac.jp/crid/1382825895359767299","@type":"Researcher","foaf:name":[{"@value":"Takao Saruta"}]},{"@id":"https://cir.nii.ac.jp/crid/1382825895359767297","@type":"Researcher","foaf:name":[{"@value":"Yoshio Goto"}]},{"@id":"https://cir.nii.ac.jp/crid/1382825895359767298","@type":"Researcher","foaf:name":[{"@value":"Masao Ishii"}]}],"publication":{"publicationIdentifier":[{"@type":"PISSN","@value":"09169636"},{"@type":"EISSN","@value":"13484214"}],"prism:publicationName":[{"@value":"Hypertension Research"}],"dc:publisher":[{"@value":"Springer Science and Business Media LLC"}],"prism:publicationDate":"2010-09-16","prism:volume":"33","prism:number":"11","prism:startingPage":"1211","prism:endingPage":"1220"},"reviewed":"false","dcterms:accessRights":"http://purl.org/coar/access_right/c_abf2","dc:rights":["http://www.springer.com/tdm"],"url":[{"@id":"http://www.nature.com/articles/hr2010162.pdf"},{"@id":"http://www.nature.com/articles/hr2010162"}],"createdAt":"2010-09-16","modifiedAt":"2023-05-18","foaf:topic":[{"@id":"https://cir.nii.ac.jp/all?q=Aged,%2080%20and%20over","dc:title":"Aged, 80 and over"},{"@id":"https://cir.nii.ac.jp/all?q=Male","dc:title":"Male"},{"@id":"https://cir.nii.ac.jp/all?q=Dihydropyridines","dc:title":"Dihydropyridines"},{"@id":"https://cir.nii.ac.jp/all?q=Kidney","dc:title":"Kidney"},{"@id":"https://cir.nii.ac.jp/all?q=Nitrophenols","dc:title":"Nitrophenols"},{"@id":"https://cir.nii.ac.jp/all?q=Organophosphorus%20Compounds","dc:title":"Organophosphorus Compounds"},{"@id":"https://cir.nii.ac.jp/all?q=Cardiovascular%20Diseases","dc:title":"Cardiovascular Diseases"},{"@id":"https://cir.nii.ac.jp/all?q=Chronic%20Disease","dc:title":"Chronic Disease"},{"@id":"https://cir.nii.ac.jp/all?q=Hypertension","dc:title":"Hypertension"},{"@id":"https://cir.nii.ac.jp/all?q=Humans","dc:title":"Humans"},{"@id":"https://cir.nii.ac.jp/all?q=Female","dc:title":"Female"},{"@id":"https://cir.nii.ac.jp/all?q=Kidney%20Diseases","dc:title":"Kidney Diseases"},{"@id":"https://cir.nii.ac.jp/all?q=Prospective%20Studies","dc:title":"Prospective Studies"},{"@id":"https://cir.nii.ac.jp/all?q=Antihypertensive%20Agents","dc:title":"Antihypertensive Agents"},{"@id":"https://cir.nii.ac.jp/all?q=Aged","dc:title":"Aged"}],"relatedProduct":[{"@id":"https://cir.nii.ac.jp/crid/1050013397240361344","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@language":"en","@value":"Calcium channel blocker in patients with chronic kidney disease"}]},{"@id":"https://cir.nii.ac.jp/crid/1360002216791600384","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@value":"The central mechanism underlying hypertension: a review of the roles of sodium ions, epithelial sodium channels, the renin–angiotensin–aldosterone system, oxidative stress and endogenous digitalis in the brain"}]},{"@id":"https://cir.nii.ac.jp/crid/1360005518347767040","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@value":"The Japanese Society of Hypertension Guidelines for the Management of Hypertension (JSH 2019)"}]},{"@id":"https://cir.nii.ac.jp/crid/1390001206447120640","@type":"Article","resourceType":"学術雑誌論文(journal article)","relationType":["isReferencedBy"],"jpcoar:relatedTitle":[{"@language":"en","@value":"3.
