There are several dermatoses characterized by sterile pustule formation at the subcorneal location such as pustular psoriasis, palmo-plantar pustulosis and subcorneal pustular dermatosis, in which often find detached fragments of the stratum corneum (SC) densely surrounded by many neutrophils. We have demonstrated that in vitro SC activates alternative complement pathway to produce C3b and C5a. Furthermore it was found that opsonized corneocytes stimulate neutrophils to cause a respiratory burst. Our hypothesis is that when SC comes in contact with serum in vivo, it activates complement through its alternative pathway to induce opsonic C3b as well as chemotactic C5a anaphylatoxin. Neutrophils accumulated chemotactically by C5a specifically interact with osponized corneocytes to cause an oxidative burst. These events are possibly involved in the formation of sterile subcorneal pustules.
