We report herein on a case of advanced carcinoma of the gingiva in a patient with Fanconi anemia (FA). The patient was a 31 y.o. female, who had been diagnosed as having FA at the age of 13. The tumor was located in the upper gingiva, involving surrounding structures including the upper lip. Wide local resection followed by free flap reconstruction was performed. The postoperative course was complicated by wound infection, bilateral thoracic empyema and subsequent septic shock. Long term intensive care was needed to improve the patient's general condition. At the time of 6 months after the operation, she is free of disease. We chose treatment with wide local resection followed by reconstruction, because of frequent recurrence after treatment and difficulty of radiation therapy and chemotherapy, and we succeeded in making the patient free of disease. However, we considered that we should have careful perioperative management, because we experienced life-threatening postoperative complications.
