Recently, effects of paroxetine for pruritus are reported, but the report to invalid cases of paroxetine is rare. We report a case treated effectively with mirtazapine for pruritus of the paroxetine invalid. <b>Case report</b>: A 56-years old woman was diagnosed cancer of head of pancreas and peritoneal dissemination.  After stenting by a plastic stent for obstructive jaundice in previous hospital, she came to our hospital. But, her total bilirubin (T-bil) were very high (9.9 mg/d<i>l</i>), and she was suffering from systemic pruritus. The NRS (numerical rating scale) score for pruritus was 9-10. Though she was prescribed an antihistamine in previous hospital, it was invalid. We had changed it to paroxetine, but pruritus were protraction two weeks later. After changing it to mirtazapine, the pruritus became NRS 1 on the next day, and recurrence was not seen subsequently. <b>Conclusion</b>: For pruritus of the paroxetine invalid, mirtazapine is important as one of the choices."},{"@language":"ja","@value":"【<b>緒言</b>】黄疸に合併する掻痒は, 抗ヒスタミン剤無効例が多く, 緩和に難渋する例がある. 近年, 掻痒に対するパロキセチンの効果が報告されているが, パロキセチン無効例への報告はまれである. 今回, パロキセチン無効の掻痒に対してミルタザピンが著効した1例を経験したので報告する. 【<b>症例</b>】56歳, 女性, 膵頭部がん・腹膜播種. 閉塞性黄疸に対してドレナージ術など行うも黄疸が遷延し, NRS (numerical rating scale) 9～10程度の, 抗ヒスタミン剤無効の全身掻痒が続いていた. 当院受診後, パロキセチンへ変更したが, 2週後にも掻痒は改善せず, 黄疸も遷延していた. しかし, ミルタザピンへ変更したところ, 翌日に掻痒はNRS 1となり, その後も掻痒の再燃は認められなかった. 【<b>結語</b>】パロキセチン無効の掻痒に対し,
