1881 autopsy cases were retrospectively studied from the view point of the natural course and treatment of cholelithiasis. Gallstones were found in 378 cases (choleecystolithiasis 302, choledocolithiasis 38, choledocho-cholecystolithiasis 38). The in cidence of Gallstone carriers, especially choledocholithiasis, increased with age. Many cases with Gallstones didn't die from cancer neither acute obustrctive suppuruative cholangitis (AOSC). But, the mortality rate from AOSC in choledocholithiasis was much higher than that in cholecystolithiasis and, cases of choledocholithiasis, died from AOSC, had showed mild attack before the last severe one.In therapy for choledocholithiasis, it was no significant difference in prognosis between operative and non-operative stone treatments.<BR>In conclusion, the presence of silent cholelithiasis in the aged is not an indication of operation. But, in cases of choledocholithiasis, careful observation is required and lithotripsy, if patients general condition permittable, is advisable.
