Background. Most elderly pneumothorax patients are treated by nonsurgical procedures. Few papers have been reported on surgery for refractory pneumothorax with COVID-19. Case Presentation. The patient was a 100-year-old man. A cluster of COVID-19 cases had occurred at his facility. He presented with a fever and decreased oxygenation and was taken to the hospital. He was suspected of having COVID-19. A polymerase chain reaction test for SARS-CoV-2 was positive, and chest X-ray revealed right pneumothorax. His COVID-19 symptoms were quite mild and quickly resolved; however, the air leakage of his right lung persisted, and the lungs did not expand. He underwent video-assisted thoracoscopic bullectomy and was discharged from the hospital without complications. In this case, a lung bulla was observed in the right lung, which was expected to have caused the pneumothorax. There have been several reports that COVID-19 causes pneumothorax. Therefore, COVID-19 may have been the cause of the pneumothorax in this case. Conclusion. Although most elderly patients are treated by nonsurgical procedures, surgery should be considered after evaluating the patient's general condition and risk of complications.
"},{"@language":"ja","@value":"背景．超高齢者の気胸は，手術以外の治療を選択することが多い．また，新型コロナウイルス感染症（COVID-19）を併発した難治性気胸に対して手術を施行したという報告はほとんどない．症例．症例は100歳，男性．入所中の施設でCOVID-19のクラスターが発生し，発熱および酸素化低下を認めたため，救急搬送された．SARS-CoV-2 PCR検査で陽性，さらに胸部X線写真で右気胸を認め，COVID-19および右気胸の診断で入院となった．COVID-19に関しては軽症で，すみやかに改善し隔離は解除されたが，気胸に関しては気瘻の改善なく，肺の拡張が乏しいため，全身麻酔下に手術を施行した．その後，合併症を起こすことなく退院した．本症例は右肺に囊胞を認め，それが破裂したことが気胸の原因と予想された．COVID-19が気胸を発症する要因になるという報告もあり，COVID-19が影響を与えた可能性もある．結論．超高齢者は手術以外の治療を選択することが多いが，全身状態や合併症リスクなどを評価した上で，手術も考慮にいれる必要がある．
