symptoms：LUTS）の従来の治療戦略は，膀胱平滑筋収縮や前立腺増殖，肥大といった下部尿路臓器そのものに対して注目されていたが，最近の疫学調査から動脈硬化の危険因子（高血圧，脂質異常，喫煙，糖尿病）とLUTSの関連性が報告され，下部尿路臓器だけでなく血流という観点から全身を治療標的とする概念が提唱されている．基礎研究においても，加齢による血管内皮機能の低下および生活習慣病等による動脈硬化から，膀胱および前立腺への血流が低下し，過活動膀胱および前立腺肥大が惹起されることが，動物モデルにて報告されている．著者らは下部尿路臓器での慢性虚血（血流低下）を改善することで，LUTS（過活動膀胱および前立腺肥大症）への改善効果が期待できるとの仮説をたてた．前立腺肥大等により誘発される急性尿閉は，尿閉および尿閉解除時に虚血再灌流障害を発生させ，膀胱機能障害を惹起する．著者らは，ATP感受性カリウム（ATP sensitive potassium：K<sub>ATP</sub>）チャネル開口薬ニコランジルをラット急性尿閉モデルに腹腔内投与した．その結果，尿閉解除時に再開する血流が，ニコランジル投与群では急性尿閉群と比べて，増加していた．また，急性尿閉群ではコントロール群に比べて，膀胱収縮力の低下および膀胱上皮でのアポトーシスの指標となるTUNEL陽性細胞の増加がみられたが，ニコランジル投与により，これらの変化をコントロールレベルまで抑制した．また，過活動膀胱モデルとされる自然発症高血圧ラット（spontaneously hypertensive rat：SHR）にニコランジルを慢性投与することで，膀胱血流増加と同時に頻尿の改善効果が得られた．さらに，SHR前立腺においては，ニコランジル慢性投与により，前立腺血流増加，酸化ストレスの減少および前立腺過形成抑制効果が観察された．以上より，ニコランジルは従来報告されていた膀胱，前立腺平滑筋への弛緩作用に加えて，下部尿路臓器の血管平滑筋弛緩作用によって大幅な血流増加を促したことで下部尿路症状が改善したことが推測される．2014年4月にホスホジエステラーゼ（PDE）5阻害薬（タダラフィル）が本邦で初めての前立腺肥大症に伴う排尿障害改善薬として発売された．PDE5阻害による一酸化窒素（nitric
