The subjects were 223 patients with StageⅡ colon cancer surgically treated between April 1997 and December 2008.  The relationships between risk factors proposed by the ASCO, NCCN, or ESMO guidelines and recurrence were examined.  The percentage of patients who met at least one of the high-risk factors proposed by the ASCO, NCCN, and ESMO guidelines was 55.6% (n=124), 89.2% (n=199), and 92.8% (n=207), respectively.  On multivariate analysis using the Cox proportional hazard model, tumor perforation, pT4 and obstruction (<i>p</i><0.01) were identified as independent significant risk factors affecting disease-free survival (DFS).  The 5-year DFS was 69.4% in patients with tumor perforation, pT4 or obstruction and 95.2％ in patients without these three factors (<i>p</i><0.01).  We could not directly accept the high-risk factors proposed by the Western guidelines in the treatment of StageⅡ colon cancer since these guidelines do not clearly identify what is more important among risk factors.  In the Japanese clinical practice, it would be necessary to select high-risk factors as examined in the present
