Solitary fibrous tumors (SFT) are uncommon spindle cell neoplasms arising from undifferentiated mesenchymal cells. They usually present as pedunculated masses attached to the visceral pleura with benign histologic features. <i><b>Case</b></i>. A 62-year-old woman presenting with cough and dyspnea was referred to our institution for treatment of a giant tumor, occupying the right hemithorax, detected on a chest computed tomography. Though the definitive diagnosis was not obtained preoperatively, the patient underwent emergency surgery since she suffered from severe dyspnea caused by compression of the heart and lung due to the giant tumor. We had to perform right pneumonectomy because of the massive tumor invasion of the lung parenchyma. Grossly, the tumor with a maximum dimension of 25 cm arising from interlobar pleura, infiltrating deeply into the lung parenchyma, and invading the truncus intermedius and pulmonary artery. Histologically, the lesion showed a proliferation of spindle cells arranged haphazardly with myxoid stroma. The tumor had increased mitotic activity, necrosis, and hemorrhage. Immunohistochemical staining showed that tumor cells were positive for CD34, bcl-2, and negative for AE1/AE3, S-100, and α-smooth muscle actin. The tumor was diagnosed as a malignant SFT. <i><b>Conclusion</b></i>. We encountered a rare case of giant SFT which presented with massive pulmonary invasion and malignant histological features. Immunohistochemical examinations were useful for definitive diagnosis.<br>"},{"@value":"<b>背景</b>．孤立性線維性腫瘍（solitary fibrous tumor：SFT）は未熟な間葉系細胞を発生母地とする比較的稀な紡錘形細胞腫瘍で，臓側胸膜から有茎性に胸腔内へ発育し，組織学的に良性であるものが多い．<b>症例</b>．62歳女性．咳と呼吸困難を訴え，胸部CTで右胸腔を占拠する巨大腫瘍を認め，治療目的で当科へ紹介された．術前に確定診断は得られなかったが，巨大腫瘍が心臓および肺を圧迫し，高度の呼吸困難を伴ったため緊急手術を行った．腫瘍は肺実質に広範に浸潤しており，腫瘍とともに右肺全摘術を行った．腫瘍最大径は25 cmで，肉眼的には葉間胸膜から発生した腫瘍が肺実質内に深く浸潤増殖し，中間気管支幹および肺動脈に浸潤していた．組織学的には紡錘形の腫瘍細胞がmyxoidな基質を伴い不規則に増殖し，多数の核分裂像を認め，また腫瘍内に出血や壊死がみられた．免疫組織染色で腫瘍細胞はCD34，bcl-2に陽性，AE1/AE3，S-100，α-SMAに陰性で，悪性SFTと診断された．<b>結論</b>．広範な肺浸潤を伴い，組織学的に悪性の像を示す稀な巨大SFTを経験した．SFTの確定診断には免疫組織染色が有用であった．<br>"}],"abstractLicenseFlag":"disallow"}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1410572171446818947","@type":"Researcher","personIdentifier":[{"@type":"NRID","@value":"9000020741452"}],"foaf:name":[{"@language":"en","@value":"Ishida
