He had undergone aortic valve replacement 1 month before admission to our hospital. Since computed tomography revealed a pseudoaneurysm in the ascending aorta, he underwent an emergency operation. An occlusion catheter was inserted into the ascending aorta via the left femoral artery, in preparation for pseudoaneurysm rupture. Cardiopulmonary bypass was established with inflow via the right femoral artery and the right axillary artery, and with vacuum-assisted venous drainage via the right femoral vein. After core cooling, we performed resternotomy. The pseudoaneurysm ruptured while we were exfoliating the adhesion around the aorta. We inflated the occlusion catheter in the ascending aorta and controlled the bleeding. We continued core cooling and ventricular fibrillation occurred at 30°C. Subsequently, we induced circulatory arrest, and selective cerebral perfusion was initiated. We inflated the occlusion catheter in the descending aorta and initiated systemic circulation with inflow via the right femoral artery. The origin of the pseudoaneurysm was the region of cannulation in the previous operation. Therefore, we replaced the ascending aorta and performed omentopexy. In this case we reported the use of a strategy involving cardiopulmonary bypass for a pseudoaneurysm in the ascending aorta."},{"@language":"ja","@value":"29歳の男性．大動脈弁置換術後に胸骨下膿瘍形成の既往があり，今回，発熱と胸痛を主訴に当院を受診した．CTを施行し，感染に伴う上行大動脈仮性瘤の診断で，緊急手術となった．仮性瘤破裂時の出血に備え左大腿動脈よりニプロオクルージョンカテーテル<sup>®</sup>（ニプロ(株)，大阪）を留置し，右大腿静脈から陰圧吸引補助脱血，右大腿動脈および右腋窩動脈送血で体外循環を開始し全身冷却のうえ，胸骨正中切開を施行した．癒着剥離時に仮性瘤が破裂し，オクルージョンカテーテルを上行大動脈で拡張し止血，全身冷却をすすめ心室細動で循環停止とし選択的脳還流を開始した．前回手術時の送血部に破裂孔を認め，上行置換術を施行し大網充填を追加した．オクルージョンカテーテルと陰圧吸引補助脱血法を使用し，短い体循環停止で手術を施行することができたので報告する．"}],"abstractLicenseFlag":"disallow"}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1410001204049023496","@type":"Researcher","personIdentifier":[{"@type":"NRID","@value":"9000006599933"}],"foaf:name":[{"@language":"en","@value":"Kuroda
