Idiopathic mesenteric phlebosclerosis (IMP) is a recently known and rare disease entity. In spite of the unique histopathology and particular location, the cause and pathogenesis of IMP remains unknown. We herein report four cases of IMP, all of whom had been taking Chinese herbs for prolonged periods of time. Case 1 was a 58-year-old male, who tested positive for fecal occult blood. This case revealed dark-colored and edematous mucosa on colonoscopy, but showed no calcification on CT. We considered the early case of IMP. Case 2 was a 69-year-old female with the complaint of abdominal pain. Colonoscopy and CT revealed no abnormality. However, after eight years, this case developed typical IMP. Case 3 was a 53-year-old female with the complaint of abdominal discomfort. CT revealed calcifications in the right hemicolon, and colonoscopy revealed dark-colored mucosa and ulcerations. Case 4 was a 48-year-old female who tested positive for fecal occult blood. Colonoscopy and CT revealed no abnormality. However, after nine years, this case developed IMP, and a right hemicolectomy was performed. We speculate that some components of Chinese herbs may contribute to the development of IMP, at least in a certain proportion of cases.<br>
