The timing and structural displacement of swallowing in lateral videofluoroscopic views during the swallowing of a 5-m<i>l</i> bolus were analyzed using the UC Davis Dynamic Swallow Study (DSS) with reference to the normative values for a 3-m<i>l</i> bolus.<br>Results: Hypopharyngeal transit time was not delayed in any patient, although oropharyngeal transit time was delayed in 3 patients.  Bolus arrival at the Upper Esophageal Sphincter (UES) occurred prior to supraglottic closure in 5 patients.  Pharyngeal constriction was decreased in 3 cases.  Motor disturbance of the oral phase, airway protection defense function, and decrease in pharyngeal constriction were seen in half the patients.<br>Discussion: This study indicated the usefulness of DSS analysis for dysphagia in Parkinson's disease patients.  In order to evaluate the diagnostic utility of DSS, the swallowing patterns of various neurodegenerative diseases need to be evaluated quantitatively using this analytical tool."},{"@language":"ja","@value":"本研究はUC Davis Dynamic Swallow Study（以下DSS）を用いて，錐体外路性の運動障害を背景としたパーキンソン病の嚥下動態を解析し，その臨床的有用性について検討したものである．<br>経口摂食を行っているパーキンソン病患者7例（男性2名，女性5名）を対象に，5 m<i>l</i>の硫酸バリウム溶液を指示嚥下した．その際のVF側面透視録画のデータを計測し，標準化されたDSS基準値と比較した．その結果，嚥下反射惹起の遅延は全例に認められなかったにもかかわらず，食塊移送時間の低下は3例（42.8％）に認められた．咽頭期では気道閉鎖の前に食道に食物が達した例は5例（71.4％）あり，咽頭収縮率の低下は6例中3例（50.0％）に認められた．解析の結果，口腔期の運動障害，気道防御機能の低下，咽頭収縮力の低下を示唆する所見がそれぞれ半数近くに認められた．中等度の特発性パーキンソン病の嚥下障害にDSS解析を適用し予備的に検討した結果，神経疾患患者の嚥下障害の解析に適応しうる可能性が示された．今後は重症度に伴う症状の変化も含め，症例数を増やしたデータの蓄積とともに，パーキンソン病以外の神経筋疾患についてもDSS解析が適用可能かを検討することが必要である．"}],"abstractLicenseFlag":"disallow"}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1410290697486589063","@type":"Researcher","personIdentifier":[{"@type":"NRID","@value":"9000262366840"}],"foaf:name":[{"@language":"en","@value":"Kudo
