Nonproteinuria subjects with type 2 diabetes often develop chronic kidney disease (CKD). To clarify the extent of renal damage in such subjects, we studied risk factors associated with renal function reduction in those with type 2 diabetes with and without microalbuminuria. Subjects numbered 638 with type 2 diabetes -513 with nonalbuminuria and 125 with microalbuminuria. Of these, 67 (13.6%) without albuminuria and 40 (32%) with microalbuminuria had Ccr lower than 60 m<i>l</i>/minute, indicating CKD. Multivariate stepwise linear regression analysis indicated that systolic blood pressure (p=0.00004), body mass index (BMI) (p=0.00010), diabetes duration (p=0.00054), and HbA<sub>1</sub>c (p=0.02606) were independent risk factors for urinary albumin excretion (UAE) exceeding 30 mg/gCr, while age (p<0.0001), UAE (p=0.00004), and waist circumference (p=0.00055) were independent risk factors for Ccr lower than 60 m<i>l</i>/minute. Risk factors for CKD between type 2 diabetes with microalbuminuria compared to those without proteinuria differed in renal function reduction. We concluded that to prevent kidney disease in diabetic patients, diabetes, hypertension, and obesity should all be managed together. Our results also indicate different etiologies for diabetic nephropathy and CKD."},{"@language":"ja","@value":"蛋白尿陰性の2型糖尿病患者における腎機能低下の原因を明らかにすることを目的とした．当院に入院した2型糖尿病患者のうち尿中アルブミン排泄量(urine albumin excretion: UAE) 300 mg/g·Cr未満であった638例を対象とした．UAE 30 mg/g·Cr以上およびクレアチニンクリアランス(Ccr) 60 m<i>l</i>/min未満と関連する因子を，年齢，性別，糖尿病罹病期間，HbA<sub>1</sub>c, 中性脂肪，HDLコレステロール，LDLコレステロール，収縮期血圧，拡張期血圧，UAE, body mass index (BMI), ウエスト周囲径，喫煙歴，飲酒歴を説明変数として，ステップワイズ重回帰分析を用いて検定した．UAE 30 mg/g·Cr以上となる因子は収縮期血圧，BMI, 罹病期間，HbA<sub>1</sub>cであった．また，Ccr 60 m<i>l</i>/min未満となる因子は年齢，UAE, ウエスト周囲径であった．腎症病期別にみたところ，Ccr 60
