A Case of Nodular Cutaneous Lupus Mucinosis

Description

A 22-year-old man presented to our hospital with joint pain and disturbance of mouth opening, which had continued for two months. He also had a two-month history of multiple subcutaneous nodules on his bilateral arms, butterfly rash of the face and muscle pain. He was diagnosed as having SLE because his condition fulfilled the diagnostic criteria for this disease. Skin biopsy revealed abundant deposition of acid mucopolysaccharide between collagen bundles in the dermis. At this time, we diagnosed the patient as having nodular cutaneous lupus mucinosis. The patient was treated with steroid pulse therapy and these eruptions were rapidly reduced. After this therapy, prednisolone and tacrolimus were started. The dose of prednisolone was gradually reduced to 20mg/day from 75mg/day, and the dose of tacrolimus from 4mg/day to 1mg/day, resulting in complete disappearance of the lesions. NCLM is a rare occurrence associated with systemic lupus erythematosus. Many researchers have suggested that the development of NCLM is associated with SLE activity.
