Bone marrow biopsy and computer tomography-assisted needle biopsy of the tumor revealed infiltration of lymphoma cells showing high-grade cytology with a mature B-cell immunophenotype and high proliferation index determined by Ki-67 immunohistochemistry. Cytogenetic studies of bone marrow specimens revealed two distinct karyotypes involving chromosomes 3, 14, and a marker chromosome. A series of fluorescence <i>in situ </i>hybridization studies of metaphase and interphase nuclei revealed a three-way translocation, t(3;12;14)(q27;p12;q32), leading to a <i>BCL6</i>-immunoglobulin heavy chain (<i>IgH</i>) fusion gene, and two distinct c-<i>MYC </i>translocations involving either the <i>IgH </i>or a non-<i>Ig </i>gene as partners. With multiple cycles of chemotherapy, the patient finally achieved complete response. We concluded that this case fulfills the criteria of doublehit lymphoma carrying both c-<i>MYC </i>and <i>BCL6 </i>translocations, and that the lymphoma be placed into the category of the WHO 2008 classification “B-cell lymphoma unclassifiable with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma\"."},{"@language":"ja","@value":"<b>症例:</b> 57歳女性．3か月前から左腰痛を自覚し次第に増強するため当院に紹介．入院時は疼痛のため仰臥位がとれない状態であった．<br><b>検査結果:</b> ヘモグロビン14.9 g/dl，血小板16.3×10<sup>4</sup>/μl，白血球5,800/μl，LDH 520 IU/l，尿酸7.8 mg/dl，可溶性IL-2R 1,010 U/ml．CT で仙骨～左腸骨，左卵巣，右大腿骨近位部に腫瘍を認め，PET/CTでこれらの病変にFDGの集積を認めた．骨髄生検と腫瘍針生検で細胞質に空胞を有する大型の腫瘍細胞を認め，CD19<sup>+</sup>, 20<sup>+</sup>, 5<sup>－</sup>, 10<sup>－</sup>, SmIgκ<sup>+</sup>, BCL2<sup>+</sup>, Ki-67≒100%, DNA index 1.11であった．<br><b>染色体・FISH解析:</b> 染色体数50~51，1つまたは2つのder(14)とマーカー染色体を認めた．<i>IgH</i>, c-<i>MYC</i>, <i>BCL6</i>プローブを用いたFISHで，<i>BCL6</i>の再構成を伴うt(3;12;14)(q27;p12;q32)転座と，der(14)上のc-<i>MYC</i>と<i>IgH</i>の再構成，またはマーカー染色体上のc-<i>MYC</i>とnon-<i>IgH</i>遺伝子との再構成を確認した．<br><b>経過:</b> R-CHOP, CODOX-M, DHAPプロトコールを順次実施し，治療開始6か月後に寛解状態に至った．<br><b>考案:</b> 本症例はc-<i>MYC</i>と<i>BCL6</i>の遺伝子再構成を伴うdouble-hit lymphomaと考えられる．WHO分類に従うとB-cell lymphoma unclassifiable with features intermediate between diffuse
