Serologic tests are very important in the diagnosis of syphilis; however, in early syphilis, test results are sometimes negative. The relationship between results of serologic tests [automated Treponema pallidum latex agglutination (TPLA) and rapid plasma reagin (RPR)] at diagnosis and time from exposure to diagnosis or time from onset to diagnosis was evaluated. In primary syphilis, the results of TPLA and RPR are not associated with time from exposure to diagnosis, but are associated with time from onset to diagnosis. In contrast, both TPLA and RPR are always positive at diagnosis in secondary syphilis. These results suggest that primary syphilis cannot be excluded, even if serologic tests results are negative.
