A 40-year-old man visited the hospital for the swelling of lymph node in the right groin and edema of the right lower limb. He had liver dysfunction, and swelling of bilateral inguinal lymph nodes; computed tomography revealed swelling of the bilateral inguinal lymph nodes and peritoneal lymphadenopathy. However, a biopsy of the right inguinal lymph node showed nonspecific inflammation; ulcerative lesion was observed in the scrotum. Rapid plasma reagin (RPR) and T. pallidum latex agglutination tests were positive; therefore, we established a diagnosis of early syphilis. Oral amoxicillin and probenecid were effective. Eight weeks after starting oral administration, the RPR test decreased by 75%. All the symptoms resolved. We conclude that syphilis diagnosis requires physical examination and history taking."},{"@language":"ja","@value":"肝機能障害と全身性のリンパ節腫脹を認めた早期梅毒の1例を経験したので報告する。症例は40歳代、男性。右鼠径部の硬結と右下肢浮腫を自覚し、精査目的に前医を受診した。血液検査で肝機能障害を、CTで両側鼠径部リンパ節と後腹膜リンパ節の腫脹を認めた為に、悪性リンパ腫などを疑い右鼠径部リンパ節の生検を施行した。生検時に陰嚢に潰瘍性病変を認め、問診にて性風俗店の利用を聴取した為に、梅毒血清反応検査を施行した。STS定量とTPLA定量が高値であり、治療目的に当院に紹介となった。肝機能障害と全身性のリンパ節腫脹を認め、また梅毒血清反応が陽性である為に、早期梅毒と診断し、AMPCとProbenecidの内服により治療を開始した。治療開始8週間後には抗体価は治療開始前の1/4以下まで低下し、治癒と判断した。梅毒の診断には、詳細な問診の聴取と適切な身体診察が必要であると考えられた。"}],"abstractLicenseFlag":"disallow"}],"creator":[{"@id":"https://cir.nii.ac.jp/crid/1410009224214567556","@type":"Researcher","foaf:name":[{"@language":"en","@value":"Kitano
