We report here a case of idiopathic azygos aneurysm that is a rare disease. A 76-year-old woman was referred to our hospital due to an abnormal chest shadow. Plain CT scan showed a mediastinal tumor. Ultrasonic bronchoscopy findings led to a suspicion of a vascular lesion, thus contrast-enhanced CT and MRI scans were performed. The lesion was shown to be located at the azygos vein arch in the shape of a lump and was diagnosed as an azygos vein aneurysm. Resection of the aneurysm was performed due to concerns regarding possible thrombosis and pulmonary embolism, though no thrombus was detected in the aneurysm during the surgery. Most patients with the disease are asymptomatic and we are permitted to follow their clinical courses, however, there are some patients who develop thrombosis and/or pulmonary embolism. In cases of azygos vein aneurysm, the surgical indication should be carefully determined case by case.
"},{"@language":"ja","@value":"稀な疾患である特発性奇静脈瘤を経験した．症例は76歳，女性．胸部異常陰影を指摘され当院へ紹介となった．単純CTで縦隔に腫瘤を認めた．超音波気管支鏡検査で血管病変が疑われたため，造影CT・MRIを施行した．病変は奇静脈と連続して上大静脈との合流部近傍に瘤状に存在しており，奇静脈瘤であった．瘤内に血栓を認めなかったが，将来的な血栓症や肺塞栓が危惧され，胸腔鏡補助下に瘤の切除を行った．特発性奇静脈瘤では無症状であることが多く経過観察が可能なことも多いが，血栓形成や肺塞栓症の報告もあり症例ごとに手術適応を検討すべきであると思われた．
