Abstract: In order to identify issues and challenges in the medical, long-term care, and welfare fields in Nayoro City with the aim of allowing elderly individuals requiring care to continue living comfortably in their familiar homes, it is essential to consider services that can address the lack of coordination in the regional comprehensive care system. This survey targeted staff involved in medical, long-term care, and welfare in Nayoro City, and 105 individuals from 14 professional roles associated with medical institutions, long-term care facilities, social welfare councils, home care support offices, and home-visit nursing stations provided responses. Assessment of regional collaboration among medical, long-term care, and welfare indicated a significant difficulty in interaction and cooperation among different professions in the community. A key challenge in promoting the regional comprehensive care system was the overwhelming shortage of services needed to support elderly individuals requiring care to live at home. Services that the respondents hoped Nayoro City University would implement included companionship for elderly individuals, monitoring of elderly people living alone, and seminars or workshops enabling local residents to contribute to the well-being of elderly individuals. Moving forward, we plan to prepare for the implementation of these initiatives.
