症例 1：16 歳，女性。下口唇右側の腫脹，疼痛のため当科を受診した。下口唇右側に腫脹，疼痛および一部痂皮の付着があり，血液検査では好中球優位な白血球上昇を認めた。皮膚細菌感染や血管性浮腫を考え，セファクロルとプレドニゾロンの内服を開始した。症状が増悪したため，翌日に再診し，超音波検査で表層への開口を伴う低エコー領域を認め，口唇膿瘍を考え切開排膿を行った。初診 4 日目に症状は改善した。培養検査からは，Methicillin-resistant Staphylococcus aureus（MRSA）が検出された。症例 2：16 歳，男性。小膿疱を伴う下口唇右側の発赤，腫脹，疼痛を認め，超音波検査で，境界やや明瞭な低エコー領域がみられた。口唇膿瘍を考え，切開排膿を行い，ミノサイクリンの内服を開始した。症状は順調に改善し，培養検査からは Staphylococcus lugdunensis が検出された。口唇膿瘍は，細菌感染によって口唇に皮下膿瘍を形成する比較的稀な疾患である。初診時に口唇ヘルペスや血管性浮腫，肉芽腫性口唇炎と誤診されることも多く，経過や所見，血液検査，超音波検査による鑑別が重要である。また，起因菌として MRSA の報告が多く，早期の切開排膿と患者背景や重症度に合わせた抗生剤投与が必要である。調べ得た限り本邦における口唇に限局する口唇膿瘍の報告は 1 例，海外でも 16 症例と多くはない。今回われわれは，若年者に生じた口唇膿瘍の 2 例を経験した。口唇膿瘍の診断と治療の際に留意すべき点も含め，文献的考察を加えて報告する。
