我々はストロー様の異物が歯冠周囲に嵌入し,一般歯科で異物と認識されずに当科へ紹介された症例を3例経験した。症例1:心疾患を持つ2歳3か月の女児で齲蝕治療を主訴に来院した。下顎右側乳中切歯の歯頸部にストロー様異物が1か月程嵌入しており,歯根長1/3程度の歯槽骨吸収が認められたが歯牙に動揺はなかった。症例2:1歳5か月の女児で下顎左側乳中切歯・乳側切歯の動揺を主訴に来院した。ストロー様の異物が3か月にわたり下顎左側乳中切歯の歯冠周囲に嵌入しており,重度な歯槽骨吸収を引き起こしていた。異物除去後も歯牙の動揺は改善しなかった。症例3:1歳1か月の男児で上顎左側乳中切歯の形成不全と齲蝕を疑って来院した。2日前に上顎左側乳中切歯が突然大きくなったという。歯冠に白色のストロー様の異物が嵌入していたが,歯周組織に傷害は認められなかった。異物による歯周組織の破壊の程度は異物の嵌入期間が長いほど重度であり,適切に診断し,すみやかに除去することが予後を良好とする上で重要である。

This report describes three children with a drinking-straw-like foreign body impacted around a tooth. The children were referred by general practitioners, without diagnoses of foreign-body involvement. Case one: A girl aged two years three months with congenital heart disease visited our clinic for caries treatment. A drinking-straw-like foreign body was impacted around the mandibular right primary central incisor for one month. Alveolar bone resorption extended to about one-third in the length of the root, but the tooth was not mobile. Case two: A girl aged one year five mouths visited our clinic because of mobility of the mandibular left primary central and lateral incisors. A drinking-straw-like foreign body was impacted around the mandibular left primary central incisor for three months, causing severe resorption of the alveolar bone. The prognosis after removal of the foreign body was not good. The mobility of the tooth did not improve. Case three: A boy aged one year one month visited our clinic because of suspected enamel hypoplasia and caries of the maxillary left central incisor. His parents claimed that the teeth suddenly became larger two days previously. A drinking-straw-like foreign body was impacted around the maxillary left primary central incisor, but no lesion of periodontal tissue was present. The severity of periodontal tissue destruction by a foreign body is directly related to the duration of foreign-body impaction. It is therefore important to make a correct diagnosis and immediately remove any foreign body to improve prognosis.
