外科系患者の疼痛治療について概説した。鎮痛薬の分類、レセプター、化学構造を説明し、患者自身が調節する鎮痛法や経皮、経粘膜鎮痛のような鎮痛薬投与の新しい方法についても述べた。これらの新しい利点は血清濃度がより一定で、より効果的かつ調節可能な鎮痛を図れることである。高いピークや一定しない血清濃度による副作用は新しいアプローチでは以前の方法に比べて起こりにくい。皮膚や粘膜を介する非侵襲的方法は患者に便利で快適なペインコントロールを提供し、結果として不安が少なく、細々とした機器もいらない。

We review the pain treatment in surgical patients on this article here. We explained classification, opioid receptors, chemical structure of analgesics. Also, gave some explanation of new routes of opioid administration (i.e.; patient-controlled analgesia, transdermal and transmucosal drug delivery system). Major advantages of these new techniques are more consistent plasma concentrations and more effective and controllable analgesia. Problems associaed with peak and rough plasma concentrations occur less frequently with the new approaches than with conventional routes of opioid delivery. The non-invasive techniques via the skin and mucosal membranes offer patients a convenient, comfortable way of controlling pain and result in less anxiety and freedom from dependence on cumbersome machinery.
