Fracture has not been recognized as an etiological factor in the development of osteosarcoma. There have been rare reports of osteosarcoma developing after fracture, and the relationship between the fracture and subsequent osteosarcoma is uncertain. By reviewing papers and reports, we examined the incidence rate ratio of osteosarcoma development in four diseases with repeated bone fractures, including congenital insensitivity to pain with anhidrosis, pycnodysostosis, osteopetrosis and osteogenesis imperfect. As an open cohort study, the incidence rate ratio of osteosarcoma development in Japanese patients with repeated bone fractures was 10.7 times higher than that of Japanese population in 47 years. This result suggests that the diseases with repeated bone fracture may be an etiological factor in the development of osteosarcoma.
