抄録

2000年4月に開始された介護保険制度は、従来の高齢者福祉が行政の措置制度であったのに対し、福祉・医療系のサービスを連携させ、効果的かつ効率的な介護サービスを提供することを基本理念としている。介護保険制度開始前の持養入所の契約は、区市町村による措置であり柔軟な対応が難しかったが、介護保険制度は、契約による多様な在宅系サービスの選択が考えられる。これらの変化を鑑み、特別養護老人ホームに入所している高齢者の実態を明らかにし、ケアのあり方、特蓑での看護職の役割を考えるための基礎的資料とするための調査を行った。

The Long-Term Care Insurance Act came in to force in April 2000 in Japan. This program is enacted to establish a framework for long-term care which is almost fully financed by public fund. Since this program started, the number of beneficiaries has increased rapidly, because they can receive home-care services as their right, with 90% government subsidy. I, therefore, have come to the conclusion that the background on nursing-home residents is changing. The purpose of this study is to clarify the situation regarding nursing-home residents. This article presents the functional and mental health status of 18 nursing-home residents(1613 in all) in the Osaka Prefecture, who live in some different institutions or use the institutions while living in their own home. The life style before they move to nursing homes was also investigated. The results showed that the residents need some form of assistance with the activities of daily life, and that one third of the residents were discovered to be in a very serious condition, after relocation. Usually nurses do not work evening or night shifts. It is important to establish a cooperative system between nurses and other care-workers.
