Self-directed learning in clinical medical education is now thought to be important because knowledge and clinical skills of medicine are markedly increasing in association with the rapid progression of medical sciences. In addition, clinical risk management, patient-oriented medical treatment and team-based medical care are required as social needs. PBL tutorials and clinical clerkship have therefore been introduced as new medical education methods in Europe and USA for learning clinical skills and manner, professionalism, communication skills and presentation methods. PBL tutorials and clinical clerkship are based on adult learning theory. When adult learners have to solve real problems, such as problems involving scenarios or patients, their motivation to learn will increase and they will finally acquire sufficient medical knowledge to solve the problems via self-directed learning. In this type of education method, the teacher is required to play a role like a coach in the learning process of students rather than one-sidedly providing students with medical information. In addition, a goal of clinical medical education is to train students to become doctors who will continue to study and learn from problems that arise in the course of patients care. In order to achieve such a purpose, the system of clinical medical education must be organized on the basis of clinical care and research, and the teacher must be engaged in both. Furthermore, reform of clinical medical education should be carried out with consideration of not only the program/curriculum but also mentorship and living.
