Recent trends involving nuclear families and the increasing age of primiparas raise the need to review maternal support during the early postpartum period. The present study investigated early postpartum support from the perspectives of intended use, actual use, and expectations of new mothers, defined as those with infants less than one year old, and expectant mothers. An online survey was conducted on 721 new and 824 expectant mothers regarding participant attributes, primary supporters, and support content, period, and location. Data were collected from 98.3% and 98.2% of the new and first-time expectant mothers, respectively. A total of 60.7% of participants did not work outside the home. A total of 37.4% of expectant mothers intended to return to their parent's home for the birth, while 44.0% of new mothers had done so. For the majority of participants in both groups, the support location was their parents' home, the primary supporter was the participant's mother, and the support period was at least 4 weeks. For both new and expectant mothers, intended and actual support content primarily comprised housework, such as cooking and cleaning. Expectations of new mothers comprised “psychological support" and “encouragement" from their husbands and “consultation", “lactation care", and “physical care" from midwives. Considerable expectations were placed on health professionals to act as supporters, indicating the need for devising a support system that provides local, continuous support throughout the stages of pregnancy, childbirth, and infant care to enable mothers to choose the support that meets their needs.
