This paper clarifies what kind of communication the care managers communicate with the medicalprofessions during terminal care, and the differences in communication between the care managers from the care worker (care-careered care managers) and those from the nursing staff (nurse-careered care managers). The result shows that when the care managers appeal to the medical professions from the viewpoint of the quality of life (QoL) of the care receiver, they are conscious of the limitation of their own medical judgment skills in medical care as well as of their self-control of communication of medical requests in order not to impinge the medical expertise and autonomy of the medical professions. On one hand,during terminal care at home, as the medical conditions of the care receiver may change suddenly, the medical professions cannot collect enough his/her medical information To support the judgment of the medical professions, the care managers are highly expected to play an important role of transmitting medical information of the receiver to the professions. For this purpose, the medical manager should learn medical viewpoints. In other words, the aim of learning medical viewpoints by the care manager is to deliver not medical requests but precise medical conditions of the receiver to the medical staff. The result of comparison between care-careered care managers and nurse-careered care managers shows that care-careered care managers insist their career of care as they can deliver the QoL of the receiver and avoid the demerits of impinging expertise of medical professions due to insufficient medical knowledge. On the other hand, nurse-careered care managers insist their career of nurse as they can discover medical information and transmit it to the medical professions.
