[要旨] 症例は70歳女性で，CTで膵嚢胞性病変を指摘され当科紹介となった。超音波検査，CT，MRCPで膵体部に壁在結節を伴う径 5 ㎝の単房性の嚢胞性腫瘍を認め，主膵管に拡張なく嚢胞との交通も認めなかった。粘液性嚢胞腫瘍の診断で癌も念頭におきリンパ節郭清を伴う膵体尾部脾合併切除術を施行した。切除標本では膵体部に厚い被膜で覆われた壁の一部に腫瘤を伴う50㎜大の単房性嚢胞を認めた。腫瘤は病理組織学的にsolid～trabecular patternの上皮性腫瘍よりなり，免疫組織化学検査でchromograninA陽性，synaptophysin陽性，MIB1 indexは 5 ～20%であり，最終的に非機能性膵神経内分泌腫瘍G2 と診断された。本症例のように嚢胞形態を示す膵神経内分泌腫瘍は比較的稀で，通常腫瘍が出血や壊死をきたし嚢胞状となるとされているが，本症例の経過から嚢胞壁より発生する膵神経内分泌腫瘍があるのではないかと推測された。

[SUMMARY] A 70-year-old woman was found to have a cystic mass at the body of the pancreas. Ultrasonography and CT showed a 50mm-diameter single locular cyst with an intramural nodule. MRCP showed no communication between the cystic mass and the pancreatic duct. Under the diagnosis of a mucinous cystic tumor of the pancreas, we performed distal pancreatectomy with splenectomy and lymph node dissection. The resected specimen showed a 50mm-diameter singlelocular cystic tumor with a thick fibrous capsule. The tumor was composed of solid and trabecular pattern of epithelial cells on pathological examination. Immunohistochemical examination demonstrated chromograninA-positive, synaptophysin-positive, and the MIB1 index at 5-20％. Final pathological diagnosis was non-functioning pancreatic neuroendocrine tumor G2 with lymph node metastases. Cyst forming neuroendocrine tumor is relatively rare like our case. It is said that hemorrhage and necrosis within the tumor form cysts. However, it was supposed that there might be pancreatic neuroendocrine tumors which occurred from the cyst wall.
