We conducted a questionnaire survey for nurses working in the community comprehensive support centers and home-visit nursing facilities in the Hokushinetsu area of Japan, and categorized the comments in open-ended questions about successful experiences. The experiences of nurses in community comprehensive support centers were categorized into six categories: “making the connection between clients and medical care”, “making the connection between clients and delivery of services”, “establishing a support system for clients through multiple professions and local communities” , “maintaining the condition of clients and their stability of daily life”, “providing stability for their family life of clients”, and “carry out early detection and early response of clients ”.
