Abstract

A case of Cowper's gland cyst is reported. This patient was thirty years old and complained of perineal mass and ejaculatory difficulties. We classified the cyst into 2 groups: the urethral type growing into the lumen of the urethra, and the perineal type growing outside the urethra into pelvic space. To date, 9 cases of the perineal type including our own and 66 cases of the urethral type of the Cowper's gland cyst have been reported in the Japanese and English literatures. In making diagnosis of a mass of the Cowper's gland, what is important is to think of the possibility of this condition when one is confronted with a patient complaining of urinary difficulties, perineal tumor, bloody urethral discharge or rectal disturbances. Careful anorectal digital examination, voiding urethrography with dilute contrast medium, and passing the panendscope under direct vision should further contribute to the clinical diagnosis of these pathologic conditions. It's also recommended to include the bulbous urethra in the routine autopsies. Treatment consists of transurethral deroofing for the urethral type cyst, and surgical excision for the perineal type cyst.
