We encountered a case treated successfully by the Letton and Wilson operation. An intractable pancreas external fistula with a pancreatic cyst due to rupture of the pancreatic main duct was caused by injury from a steering wheel in a traffic accident. A 48-year old man suffered a heavy blow with a steering wheel in the epigastric region, and was admitted on an emergency basis in another hospital. He suffered fever and jaundice, and a cyst of the pancreas body was formed. Symptoms improved after puncture and drainage, but outflow of pancreatic juice continued for two months, so he transferred to our hospital. We operated 74 days after the traffic accident. We were able to do pancreaticojejunostomy (Letton and Wilson method), because we could confirm the tail side of pancreatic main duct. After operation, no complications or diabetes mellitus occured, and he recovered completely.
