Description

Minimal change nephrotic syndrome (MCNS) is associated with several in vivo and in vitro abnormalities of the immune system, including an imbalance of cytokines. Although several studies were conducted to characterize the cytokine profile in patients with MCNS, a definitive universal pattern was not identified. To elucidate the comprehensive cytokine profile that might be involved in the mechanism of proteinuria in patients with MCNS, we used a cDNA array to analyze mRNA expression of cytokines in peripheral blood mononuclear cells (PBMC) obtained from paired (onset and remission) blood samples from five pediatric patients with MCNS. Cytokine mRNA expression at onset and in remission was compared. Selected genes were further confirmed by RT-PCR and real-time PCR analyses. Both Th1 and Th2 cytokines were up-regulated, but real-time PCR analysis showed a predominance of Th2 activity in MCNS patients at onset as compared to during remission. In conclusion, T cell activation evolving toward a Th2 profile may be associated with the pathogenesis of MCNS.
