With the rapid progress of various diagnostic methods and information technology in medical care, the very important and basic techniques of history taking and physical examinations are receiving diminished attention. A human relation which is created on the basis of conversation is essential between the medical staff and the patient. A good human relation may start with the recognition of and sympathy for the patient's complaints and conditions, and is matured through many explanations, questions and answers such as done in the process of obtaining informed consent. On the other hand, there are well-informed patients who obtain a variety of medical information on EBM, guidelines and so on. Recently books have been published on the subject of "kanjagaku" or "expert patients". These books state that the first thing to obtain is a good human relation between the patient and the doctor. Medical doctors must learn and develop communication skills, provide patients with easily understood explanations, and strive to produce a familiar atmosphere which is conducive to accepting questions and talking supportively to patients.
