The USS Liberty was attacked by Israeli Forces on June 8, 1967 during the Six-Day War. The combined air and sea attack killed 34 (naval officers, seamen, two Marines, and one civilian) and wounded 170 crew members.

Both the Israel, and U.S. governments conducted inquires and issued reports that concluded the attack was a mistake due to Israeli confusion about the identity of the ship. Shortly before the war began, the USS Liberty was ordered to proceed to the eastern Mediterranean to collect electronic intelligence (or spying activities). After the war started, several messages were sent to the Liberty to increase its distance from the Egyptian and Israeli coastlines, but due to a Standard Operating Procedure (SOP) error, these messages were not received until after attack.

Israel reports concluded the attack on the Liberty was carried out because it was mistaken for an Egyptian supply ship and because of failures of both Israeli and the U.S. commands.

The Johnson Administration accepted this conclusion because of the powerful Israel lobbies and even Congress was reluctant to have an inquiry as an election was coming in December. The President did not want this incident became the diplomatic problem between the USA and Israel. At government level, the Liberty incident classified as a mistake and money was paid to the families of the dead and wounded.

On July 2, 2003, the National Security Agency released copies of the recordings made by the EC-121 and their translations. The English transcripts of the released tapes indicate that Israel still believed it had hit an Egyptian supply vessel even after the attack had ended.
