Abstract
This literature review provides some of the effective nursing practice guidelines applied Transtheoretical Model to enhance changes on exercise and dietary behavior among community-dwelling people. Transtheoretical Model is a systematic model that consists of six stages, ten processes of change, decisional balance and self-efficacy. The evaluation studies addressing exercise or dietary program in longitudinal cohort were reviewed. Stage-matched nursing practices were also discussed.
