ヒトサイトケラチン19に対する2種のマウスモノクローナル抗体,KS 19.1とBM 19.21を用いたワンステップサンドイッチ固相法radioimmunoassay (RIA)を使用して,健常日本女性および男性の血中cytokeratin fragment 21-1 (CYFRA 21-1)を測定,正常値およびカットオフ値(平均値+2SD)を検討した。また,月経周期における血中値の変動,さらに妊娠時血中値の変化,および陽性率を妊娠初期,中期,後期に分けて検討した。分娩時,母体血,羊水,臍帯動静脈血,胎盤後貯留血液および産褥3日目の母体血を採取,各検体におけるCYFRA 21-1を測定し,その産生源および血中値半減期についても考察を加えた。著者らはすでにCYFRA 21-1の婦人科悪性腫瘍に対する腫瘍マーカーとしての意義を報告したが,本論文は産婦人科領域における新しいサイトケラチン関連腫瘍マーカーのカットオフ値,月経周期および妊娠・分娩時における血中値変動等,基本的なデータに関する報告である。

Two murine monoclonal antibodies, KS 19.1 and BM 19.21, have recently been established against a human variant of cytokeratin 19. Using these monoclonal antibodies, a solid phase radioimmunoassay (RIA) based on the simultaneous sandwich principle was constructed. With the use of this RIA, we measured cytokeratin fragment 21-1 (CYFRA 21-1) concentrations in serum and amniotic fluid obtained from normal, healthy, nonpregnant or pregnant Japanese women (NHNJW or NHPJW). The aim of this study was to investigate the serum CYFRA 21-1 cut-off value in NHNJW, fluctuations of the CYFRA 21-1 level during the menstrual cycle, changes of the maternal CYFRA 21-1 level with gestation, and the CYFRA 21-1 levels in the umbilical blood and amniotic fluid. The present study thus obtained first fundamental data on CYFRA 21-1, a newly established cytokeratin-related tumor marker, in normal, healthy nonpregnant or pregnant women.
