外来通院するがん患者の主体性を活かす外来看護実践方法

The purpose of this study is to clarify practical methods of outpatient nursing taking advantage of their independence in outpatients with cancer. The subjects were 10 nurses working at the outpatient section who had been evaluated to practice nursing methods taking advantage of outpatient's independence. Through semi-structural interviews, the following results were obtained. 1. As practical methods of outpatient nursing to support the patients to find out own will and desirable condition, seven practical methods were clarified, including directly asking patient's intention, encouraging the patient to talk for finding out own value, encouraging the patient to deeply consider after informing that various ways of thinking are allowable, providing so appropriate information according patient's situation that the patient can define own intention, etc. 2. As practical methods of outpatient nursing to support the patients to solve problems by own way toward patient's desirable condition, 11 practical methods were elucidated, including recognizing that the patient him/herself makes efforts toward own desirable condition and then notifying it to him or her, informing of other solutions as paying regard to desirable conditions which the patient thinks of after recognizing patient's effort,presenting practicable methods in patient's living and also considering of the practicability, setting chances to share information among physicians and patients for the patients to be with desirable conditions which the patients themselves think of etc. 3. 20 practical methods those were not included in above 1 and 2 were clarified. It was shown that those20 practical methods were practical methods to make the basis of outpatient nursing to support the patients to find out own will and desirable condition and to support the patients to solve problems by own way toward patient's desirable condition.

近年,外来通院するがん患者が主体性を発揮して自分らしく治療や在宅療養に取り組むことを支援する看護が重要視されているが,その実践方法は明らかにされていない。本研究の目的は,外来通院がん患者の主体性を活かす外来看護の実践方法を明らかにすることである。外来通院がん患者の主体性を活かした看護を実践していると評価される10名の外来看護師を対象に半構造化面接を行い,以下の結果を明らかにした。1.患者が自己の意思やありたい姿を見いだすことを支援する外来看護の実践方法として7つの実践方法が明らかになった。患者の意向を率直に尋ねる,患者が自身の存在価値を見いだせるように語りを促す,考え方はいろいろあってよいことを伝え熟考を促す,患者が意向を明確にできるように患者の状況に応じて情報を提供する等。2.患者がありたい姿に向かって自分らしいやり方で問題解決に取り組み解決することを支援する外来看護の実践方法として11の実践方法が明らかになった。自分のありたい姿に向かって患者自身が取り組んでいることを認め伝える,患者の取り組みを認めた上で患者のありたい姿を尊重しながら他の問題解決方法を伝える,患者や生活の中で実行できる方法を提示し実施できそうかどうか共に考える,患者がありたい姿でいられるよう医師と患者が情報共有できる機会を設定する等。3.上記1および2に含まれない実践方法として20の実践方法が明らかになり,それらは,患者が自己の意思やありたい姿を見いだすことを支援する外来看護および患者がありたい姿に向かって自分らしいやり方で問題解決に取り組み解決することを支援する外来看護の基盤をつくる実践方法と考えられた。
