成人看護学実習において単一医療機関で行われている在宅療養移行支援の見学実習を取り入れた学習成果

[目的]成人看護学実習に単一の医療機関で行われている在宅療養移行支援の見学実習を取り入れたことによる学習成果を明らかにした。 [方法]成人看護学実習を履修した4年生64名を対象とした。在宅療養移行支援の見学実習レポートの記述とアンケートの回答を,テキストマイニングと質的統合法(KJ法)を用いて分析した。 [結果]見学実習レポートの記述において全ての見学場所で共通していた語は「患者」「看護師」「家族」「必要」「入院」「支援」「情報」「生活」であった。学生の78%が見学実習の学びを病棟実習での受け持ち患者への看護に活かせたと回答し,その内容として【療養生活継続のための看護展開:生活者の視点で地域に戻る患者を支援】などの5つが抽出された。 [結論]学生が多職種連携や退院支援について学んでいることや,見学実習が受け持ち患者への看護に活かされていた可能性が示され,見学実習を取り入れた臨地実習の学習成果が明らかとなった。

Purpose : This study identified the learning outcomes of adult nursing practicum that incorporated observation of transitional care provided within a single medical institution. Methods : Participants were 64 nursing students enrolled in the adult nursing practicum course. Observation reports were analyzed using text mining, and questionnaire responses were analyzed using the qualitative synthesis method of KJ Ho. Results : The analysis showed that the following words were commonly used in describing all the observation sites : “patient,” “nurse,” “family,” “need,” “hospitalization,” “support,” “information,” and “life.” Seventy-eight percent of the students reported that they were able to utilize the knowledge they had learned from the observation visits in providing nursing care for patients they were assigned to. The descriptions were summarized into five elements including ‘Development of nursing care to continue life during medical treatment : Assisting patients in returning to the community from the viewpoint of the patients.’ Conclusions : Students perceived patients as people living in the community and learned about interprofessional collaboration and discharge support. The analysis of the questionnaire data showed that the experience of the hospital observation visits was utilized in the nursing care of the patients they were assigned to, suggesting that the nursing practicum incorporating hospital observation visits was effective.
