Description

We reported a case of isolated ACTH deficiency with abnormal response of HGH to TRH test. No appreciable changes in plasma GH levels after TRH stimulation have been observed in normal subjects, whereas acute GH release hes been reported in acromegaly, primary hypothyroidism, diabetic neuropathy and other pathophysiological states. There is no report of GH release after TRH stimulation in isolated ACTH deficiency. In addition, the abnormal response remained after hydrocortisone supplementation. Etiology of isolated ACTH deficiency in this patient was demonstrated to be empty sella by MRI.
