小児外科における卒後臨床研修プログラムを改革するために, 医学生の卒後研修に対する意識をアンケート方式により調査した。卒業後の進路はプライマリー・ケアに重点を置いた家庭医を希望する学生(家庭医群)が33%(18/55), 将来専門的な医療を行いたいと考えている学生(専門医群)が60%(33/55)であり, 千葉大学の医学生は全国的な平均と比較して専門医志向が高かった。希望する初期研修方式は, ストレート方式11%(6/55), ローテイト方式40%(22/55), スーパーローテイト方式47%(26/55)であり, スーパーローテイトを含むローテイト方式が全体の87%を占め, 卒後初期研修方式としてはローテイト方式が多くの医学生に支持されていた。この傾向は家庭医群のみならず専門医群においても認められ, 専門医群の87%(29/33)がローテイト方式による卒後初期研修を希望していた。卒後, 千葉大学において希望する初期研修が受けられると考えている医学生の割合はわずか49%(26/53)であり, その主な理由として千葉大学には希望する初期研修プログラムが無いことがあげられていた。この傾向は外科系希望者で顕著であり, 彼等の50%はすでに千葉大学以外の施設での初期研修を考慮していた。将来的に診療, 研究に必要な人材を確保する意味でも, 長期的な展望にたった医学生にとっても魅力的な卒後研修プログラムの作成が必要である。

We evaluated the awareness of medical students concerning the present post-graduate training (PGT) system with the object of reforming the PGT program at the department of pediatric surgery in Chiba University (CU) School of Medicine. The type of medical doctors that the students preferred to become after graduation were specialists (60%, 33/55) and family physicians (33%, 18/55). The systems of PGT that they selected as ideal .were super-rotation (47%, 26/55), rotation (40%, 22/55) and non-rotation (11%, 6/55). Twenty-nine of 33 (87 %) students preferring specialists selected rotation including super-rotation as their ideal PGT. The proportion of students who wanted to undertake PGT at CU hospital (CUH) was only 49 % (26/53), the main reason for which being that CUH did not have their ideal PGT program. This tendency was more obvious in the students preferring to specialize in surgery. Fifty percent of students had already decided to. undertake PGT in institutes including other university hospitals outside of CUH. In future, it is likely that the number of students who undertake PGT at CUH will decrease and that human resources for medical practice and research at CUH will be depleted. It is, therefore, necessary to prepare an ideal PGT program for students.
