Description

A case of intracranial complication was reported, which was suspected to be a otogenic brain abscess. The patient was a 32 years old male and admitted to our clinic with complaints of headache, right otorrhea and fever. With a diagnosis of otogenic meningitis, he had received a medical conservative therapy and improved without a slight headache, however two weeks after the discharge, the same symptoms developed with a disturbance of conciousness. The CT scanning revealed a large space occupying mass in the right middle cranial fossa. Considering the risk to herniation and not denying a doubt of brain tumor, we decided to treat by administration of antibiotics and control of intracranial high pressure in co-operation with neurosurgeons. The intracranial mass decreased markedly, but for reasons of persistencies of a slight headache and mild fever, the radical mastoidectomy of the right ear was performed. Following the operation, the patient rapidly recovered satisfactorily without any pathological findings on the brain CT scanning. Five months after the operation, the patient showed no trouble due to intracranial lesion with a complete epithelization of the right middle ear. Following the development and imprudent uasage of antibiotics, the symptoms of otogenic brain abscess had been masked, therefore the diagnosis had been difficult to be done, however, the development of CT scanning makes it easier, accurate and particular in the recent time.
