Objective: Early detection and early prevention of dementia are important for older people, though no screening methods have been established to easily detect the disease during the pre-clinical stage. We developed a new check sheet for detecting dementia at a very early stage to capture changes in daily life that are closely related to cognitive decline. We conducted The Questionnaires for Earlier Stage of Dementia (Q-ESD) for older patients without cognitive decline, and examined the scores' relationship with scores on the Tokyo Metropolitan Institute of Gerontology Index of Competence (TMIG-IC). Methods: The Q-ESD and TMIG-IC were conducted at 13 facilities in Japan for older people, using a simple dementia screening method: Tokyo Primary Questionnaires for Dementia (TOP-Q) score of ≤1 and no diagnosis of cognitive decline, and 4 months after the initial survey. We re-examined it later. Results: There were 134 participants (male/female = 25/109) with a mean age of 75.2±12.0 years and a mean Q-ESD score of 25.0±7.0 points. There was a negative correlation between the total scores of the Q-ESD and TMIG-IC. Participants were divided into two groups: Q-ESD improvement/maintenance during the period or Q-ESD worsening. The higher ADL on the initial survey was significantly lower in the worsening group (P<0.05). Conclusions: The Q-ESD detected early decline in vital functions, suggesting a high Q-ESD score may be a preliminary indicator of developing dementia in the future.
