Spontaneous rupture of the deltoid muscle associated with massive tearing of the rotator cuff

ny a ew reports of a spontaneous rupture of the deltoid are in the literature. We report two cases of spontaneous rupture of the deltoid in association with a massive rotator cuff. This combination of the deltoid muscle rupture with a massive rotator cuff tear resulted in a dramatic limitation of shoulder elevation. CASE REPORTS Patient 1. A 71 -year-old farmer had had right shoulder pain for 5 years. Two years previously he had had severe right shoulder pain while he was working in the field, without any major trauma. At that time he recognized deformity of the right upper arm, and he was unable to actively elevate his right arm. He took analgesics and was treated with physiotherapy without significant recovery. He visited our clinic, and on physical examination there was a marked gap in the middle belly of the deltoid muscle (Figure 1). The range of active elevation of the right shoulder was less than 80”; passive elevation was 160". No neurologic deficits were present. Roentgenograms demonstrated sclerotic change of the greater tuberosity and an acromial spur. Arthrography showed a massive rotator cuff tear with the radiopaque contrast material leaking into the subdeltoid bursa and a discontinuity of the deltoid muscle (Figure 2). Magnetic resonance imaging demonstrated discontinuity of the deltoid muscle on the lateral side of the greater tuberosity
