Description

[Purpose] To study the relationship between the timing of surgery and the prognosis concerning type of disease and binocular function in many adult surgical strabismus cases.

[Methods] We retrospectively studied binocular function of 2,689 surgical cases from 2,328 adult strabismus patients who underwent day surgery under local anesthesia from August 2003 for 11 years. Retinal correspondence, suppression and stereo acuity were evaluated.

[Results] 1,604 exotropic and 428 esotropic patients were included in this study, meaning 78.9 % of patients suffered exotropia. In exotropia, the majority was 1,037 intermittent exotropia patients, followed by sensory exotropia and consecutive exotropia. In esotropia, 175 patients were congenital and almost the same 172 patients were divergence palsy. In vertical strabismus and cyclotropia, the major cause was 161 superior oblique palsy patients, followed by dissociated vertical deviation. 1,157 patients (72.1%) showed normal retinal correspondence including dual retinal correspondence. Only one case showed abnormal retinal correspondence with paradoxical diplopia after surgery, the others showed lack of retinal correspondence.

[Conclusions] In adult strabismus patients, the purposes of treatment were cosmetic purpose without double vision. We showed that abnormal retinal correspondence, which the surgery was contraindication because of paradoxical diplopia after the surgery, was very rare. Therefore, we suggested the strabismus surgery could be performed not only in childhood but also in adulthood.
