A 52-year-old female was referred to our hospital by her family doctor complaining of left ear fullness and intractable otorrhea. We started anti-tuberculosis drug therapy due to the presence of active pulmonary tuberculosis. This was effective for treatment, and surgery was not needed. A histopathological examination of the granulation from the external auditory canal was useful in making the diagnosis of tuberculous otitis media (TOM). This case is considered to be a case of secondary TOM, in which an infection with the tuberculloid bacillus occured via the external auditory meatus because the patient had a habit of scratching her ear with a toothpick.<br>TOM is now a rare cause of chronic infection of the middle ear and mastoid. Nonetheless, it should be strongly considered in patients with remedy-resistant otorrhea in order to avoid complications such as facial nerve paralysis and progressive hearing loss.
