Two Cases of Tobacco Dermatitis

Description

Two patients with tobacco dermatitis were treated. One had a primary irritant contact dermatitis and the other an allergic contact dermatitis. Case 1. In a 50-year-old woman, itchy papuloerythematous eruptions occurred on the face and back of the hands after picking tobacco buds. A patch test with raw tobacco leaf was positive after 48 hours. Case 2. A 70-year-old farmer, who had worked in the tobacco fields for over 50 years, had recently developed severe contact dermatitis on the exposed areas, while harvesting tobacco leaves. The lesions improved after harvesting was completed. Patch tests with raw and dry tobacco leaves were positive. Patch tests with aqueous and with ether extracts of dry tobacco leaves also were positive, the latter showing a stronger reaction than the former. The reaction with ether extract of dry tobacco leaves persisted for one week. Twenty-two control subjects were tested with the same extracts, and reactions were negative. Thus a genuine hypersensitivity to the tobacco leaf itself may occur.
