Abstract

A 74-year-old Japanese female was referred to us for further evaluation of a 2-year history of a slightly raised tumor on the lateral aspect of the left thigh. The tumor was 30×25 mm in diameter and ulcerated in the center with surrounding papules. The tumor was surgically removed, and a sentinel lymph node biopsy was performed. Histology showed that the tumor cells, some of which had large bizarre nuclei, invaded from the epidermis to the deep dermis forming immature ductal structures. Immunohistochemically, the tumor cells were positive for EMA and exhibited CEA-positive dot-like potential lumens in single cells. The tumor was diagnosed as eccrine porocarcinoma with metastasis to the left inguinal lymph nodes. A lymphadenectomy was done subsequently. An immunohistochemical study of the keratin expression suggested that the tumor cells were differentiated into intradermal ductal cells.
