Abstract

A 61-year-old male visited our clinic who had gout for the past 20 years and was not treated. Fifteen years previously, subcutaneous nodules appeared in both feet and gradually increased in size over time. Subcutaneous nodules also appeared in the fingers of both hands and the left elbow, and later in the left knee. A skin biopsy was performed and the nodules were diagnosed as gouty tophi. Radiography of the fingers revealed that bone destruction had occurred at the sites of the subcutaneous nodules. When the severity of gouty tophi increases, they may cause bone and joint destruction. Evaluation of bone destruction is important in cases of chronic gouty tophus.
