A Case of Chronic Radiation Ulcer Accompanied by Fatal Arterial Bleeding

Abstract

A 91-year-old woman was admitted to our hospital because of massive bleeding from the chronic cutaneous ulcer in her left chest. She was irradiated by the atomic bomb explosion at Nagasaki when she was 23 years old, and at 43 years old she had left breast cancer, which was treated by surgery and radiotherapy. At about 56 years old, radiation dermatitis occurred in her left chest, and she had been treated in our hospital since the age of 81 years under a diagnosis of chronic radiation ulcer. During follow up, bleeding occurred several times, but she did not accept surgical treatment. After the admission,angiography revealed pseudoaneurysm of the left internal mammary artery, and transcatheter arterial embolization (TAE) was performed. Although pressure dressing and blood transfusion were performed, the bleeding repeated. When she was transferred to another hospital, fatal bleeding occurred and she died. Apart from surgery, low intensity laser irradiation or treatment with epidermal growth factor are recommended as less invasive treatments for chronic radiation ulcers.
